Rodrigo Posted August 15, 2016 Author Share Posted August 15, 2016 Challenging Traditional Cardiovascular Risk Assessment Poster presentation from The Ancestral Health Symposium 2016 To summarize: This presentation discusses the primary drivers of heart disease and why traditional cardiovascular risk assessment, based on Framingham methodology is inadequate. Framingham methodology is distracting to most healthcare professionals as it does not address the true root causes of heart disease including metabolic disease and hormonal dysregulation. Proper tools that more accurately predict risk including the Dr. Joseph R. Kraft five hour insulin assay and the heart calcium scan (CT-scan of the heart) are discussed. Importantly, we also discuss how to take proper action to reduce ones’ risk. The ‘Muddy Waters of Framingham’ are murky and this presentations serves to provide clarity. Keto-adaptation is associated with profound benefits in individuals with pre- and type-2 diabetes, and there is emerging evidence it may benefit endurance athletes. Men samtidigt är det inte alla feta som har högt blodtryck, höga blodfetter eller insulinresistens. Därför har en del forskare trott att viss typ av fetma kan vara hälsosammare och att de personerna inte har en högre risk för fetmarelaterade sjukdomar än normalviktiga. Men tyvärr verkar det inte vara så visar en ny svensk studie. – ”Fat, but fit” har man sagt ibland. Men så enkelt verkar det inte vara. De här resultaten talar för att även de med hälsosam fetma har en ökad risk för både typ 2-diabetes och kardiovaskulära sjukdomar på lång sikt jämfört med normalviktiga individer, säger Mikael Rydén, överläkare och professor i klinisk och experimentell fettvävsforskning vid Karolinska institutet och en av forskarna bakom den nya studien som publicerats i Cell Reports. Han och hans forskarkollegor jämförde hur fettvävnaden hos 17 friska normalviktiga svarade på insulin med fettvävnaden hos 51 feta personer. De feta delades upp i två grupper, de som hade en så kallad hälsosam fetma och de som hade en ohälsosam fetma. Alla gener kartlades också. När en vävnad svarar dåligt på insulin kallas det insulinresistens. Det är starkt kopplat till utveckling av typ 2-diabetes. Trots att personerna som klassats som hälsosamt feta var insulinkänsliga hade de på gennivå alltså en respons som nästan var identisk med de personer som var insulinresistenta. – Vi trodde att vi skulle kunna bekräfta att det fanns så kallad snäll fetma. Men gensvaret på insulin var väldigt likt i de två grupperna med feta och skilde sig markant från de normalviktiga. Det stämmer även med stora epidemiologiska studier som kommit på senare tid som visar att personer med ”hälsosam fetma” också har en ökad risk för typ 2-diabetes och hjärt-kärlsjukdom, säger Mikael Rydén. Forskarna tolkar resultaten som att det är farligt att vara fet, oavsett vilken typ av fetma man har. – Det kanske inte är det människor vill höra. Det finns läkare och forskare som trott att feta som inte har diabetes eller hjärt-kärlsjukdom inte behöver följas upp lika noga. Men det här talar för att alla som är feta måste få åtgärder mot sin övervikt för att minska risken för följdsjukdomar, säger han. Men det finns några osäkerheter. Personerna med fetma hade i snitt ett BMI på nästan 40. BMI för normalvikt ligger mellan 18 och 25 medan gränsen för fetma ligger på 30. Det är därför inte säkert att resultaten gäller för alla personer som har ett för högt BMI. Dessutom har forskarna bara undersökt fettvävnaden. Det finns andra vävnader som har betydelse vid utveckling av insulinresistens, men de är svårare att undersöka genom vävnadsprov. De feta personerna i studien har nu genomgått viktreducerande kirurgi.
